San Juan Healthcare’s medical director Dr. Burk Gossom, certified diabetes educator Joann Mayo, and administrative assistant Susan Murfin recently attended a three-day conference in Washington, D.C. as part of a two-year federal grant supporting San Juan Healthcare’s diabetes management program.
All 60 facilities in attendance displayed poster board presentations outlining their current quality improvement efforts in diabetes care.
San Juan Healthcare’s diabetes registry program supports the care of the clinic’s diabetes patients. The registry program allows health care providers to better track patient progress toward diabetes targets, to alert patients when they are due for a visit with their health care provider and see if new care processes are improving patient health.
The diabetes registry supports the care of about 150 patients who receive their primary diabetes care at San Juan Healthcare.
Grant participation requires monthly telephone conferences with the other nationwide recipients and data submission regarding performance in standard diabetes measures. In 2010, San Juan Healthcare team members will be attending two additional grant-funded national conferences for further learning and sharing with national peers.
In 2010, the San Juan Healthcare team will begin a small-scale trial of adding cardiovascular care to the registry supported processes.
