The following forms can be picked up from the elementary school office

Wednesday, March 23 from 9 a.m.-11 a.m. and 1 p.m.-4 p.m.

You are welcome to sit in the cafeteria to fill out your packet. If you have any questions; assistance will be available. Or call 360.370.7301.

Please bring in your child’s birth certificate when you come in, a copy will be made for your child’s file.

Also available for your reference, information regarding required vaccines.

> 2 doses MMR

> 2 doses Varicella

*Vaccine Doses may be fewer than listed depending on your child’s situation.
