Friday Harbor Elementary School staff invites all parents of children who will be 5 years old on or before Aug. 31, 2019, to meet with school staff and begin the kindergarten registration process, if they haven’t done so already. Bring your child’s immunization record and birth certificate to your appointment. To begin school your child must be in compliance with Washington state vaccination requirements, or have a valid exemption on file.

Required immunizations:

• Five doses of Diphtheria, Tetanus, Pertussis.

• Four doses of Polio.

• Two doses of Measles/Mumps/Rubella.

• Three doses of Hepatitis B.

• Two doses of Varicella OR date of disease verified by Healthcare provider.
